What brings on a sore throat?

The overwhelming majority of sore throats come from viruses — the same bugs that cause colds and flu. Bacterial causes are far less frequent, the main culprit being Streptococcus (“strep throat”). Tonsillitis (which may be viral or bacterial), glandular fever, and simple irritation from smoke, dry air or acid reflux can all play a part too.

No single feature settles it, but the whole picture points the way.

Leans viral

• A sore throat alongside a cough, a runny or stuffy nose, and a hoarse voice.
• Mild to moderate soreness that eases over a few days.
• Seen at any age, and the typical story with a cold.

Leans bacterial (strep throat)

• A sore throat with little or no cough or runny nose.
• Fever, white patches or pus on the tonsils, and tender, swollen neck glands.
• Comes on quickly, and more common in children and teenagers.

Soothing a sore throat at home

For most sore throats, straightforward home care is all that is required while it runs its course:

• Pain relief — paracetamol or ibuprofen (if they suit you) is the single most useful thing for the pain.
• Keep your fluids up — warm or cool drinks, whatever feels most soothing.
• Comfort the throat — medicated lozenges, or a salt-water gargle for adults and older children.
• Rest, and stay away from smoke and other irritants.

Antibiotics do nothing for a viral sore throat and are not a painkiller — good pain relief matters far more from day to day.

So when are antibiotics actually needed?

Antibiotics are not a routine treatment for a sore throat, since most are viral and even plenty of strep throats resolve unaided within a few days. A doctor weighs the Centor/FeverPAIN score together with examining you. Broadly:

• Low score — antibiotics are not advised; self-care is the way to go.
• Medium score — the doctor may offer a “back-up” (delayed) prescription to use only if you do not pick up, or take a throat swab.
• High score — antibiotics may be considered, particularly if you are more unwell or at higher risk.
