Paronychia & Nail Infections: Sore, Swollen Nail Folds Explained

A red, swollen, throbbing fold of skin beside a fingernail or toenail — often after a bitten nail, an overdone manicure or a hangnail — is paronychia, one of the most common infections of the hand. Caught early it settles with simple treatment; left brewing it can form an abscess that needs draining. A registered Irish doctor can assess it by video today for €34.99.

Acute paronychia — the sudden, painful one

  • Develops over hours to days beside one nail: redness, swelling, warmth and a throbbing ache.
  • Usual triggers: nail biting, picking hangnails, aggressive cuticle trimming, an ingrown edge or a small injury.
  • A white or yellow pocket of pus at the nail fold means an abscess is forming.

Treatment

  • Early (no pus): warm soaks 3–4 times a day for 10–15 minutes, paracetamol or ibuprofen for pain, and often an antibiotic — topical or oral — which the doctor can prescribe after assessment.
  • Visible pus: the collection usually needs releasing — a quick, very relieving in-person procedure. Antibiotics alone rarely fix a formed abscess, so the doctor will direct you to the right place rather than let it linger.
Same-day, in-person care needed if redness is spreading up the finger or hand, you have a fever or feel unwell, the fingertip pulp is very swollen and tense, movement of the finger is painfully limited, or you have diabetes or poor circulation — hand infections can escalate quickly and are not a wait-and-see problem. Severe cases: Emergency Department.

Chronic paronychia — the slow, stubborn one

  • Weeks to months of puffy, tender nail folds — often several fingers, and classically in wet-work hands: cleaning, catering, hairdressing, healthcare, parenting small children.
  • The cuticle seal is lost, letting water, irritants and yeast (candida) keep the fold inflamed; nails can grow ridged or discoloured.

Treatment

  • Keep the hands dry — cotton-lined gloves for wet work, gentle handwashing, no cuticle picking or manicure trauma while healing.
  • Prescription creams — typically a steroid, an antifungal, or a combination — used for several weeks while the cuticle barrier regrows.
  • Expect a slow fix: the fold heals over weeks, and protection from water is as important as any cream.
Nail habits that prevent the next one: don’t bite nails or tear hangnails (clip them), push cuticles back gently rather than cutting, keep manicure tools clean and personal, moisturise after handwashing, and cut toenails straight across to avoid ingrowing edges.

How the online consultation works

  1. Book a time above — same-day appointments are usually available, seven days a week.
  2. Show the nail by video — close-up photos from a couple of angles help the doctor judge whether pus has collected.
  3. Get your plan — antibiotics or combination creams prescribed electronically where appropriate, soak-and-care instructions, and a straight answer if it needs in-person drainage.

Frequently asked questions

Will a nail infection clear on its own?

A very early, mild one can settle with diligent warm soaks. But increasing pain, swelling or any pus pocket means it’s progressing — treatment at that stage prevents an abscess and a procedure.

Should I squeeze the pus out myself?

No — squeezing or stabbing it at home pushes infection deeper and risks the tissues around the nail. If pus has collected, proper drainage is quick, clean and immediately relieving.

Can an online doctor prescribe antibiotics for it?

Yes — after seeing the nail by video and photos, a registered Irish doctor can prescribe topical or oral antibiotics where clinically appropriate, sent electronically to your pharmacy, and will tell you plainly if it instead needs hands-on drainage.

Why do my nail folds stay puffy for months?

That’s the chronic form — a lost cuticle seal plus constant wet work keeps the fold inflamed, often with yeast involved. It responds to weeks of dryness, glove protection and prescription creams rather than a single antibiotic course.

Is it related to ingrown toenails?

Closely — an ingrowing nail edge digging into the fold is a common trigger for toe paronychia. Straight-across trimming, roomy footwear and early care for any ingrowing edge prevent repeats.

I have diabetes — does a nail infection matter more?

Yes. Infections in hands and feet escalate faster with diabetes and deserve earlier treatment and a lower threshold for in-person review — don’t wait it out.
