What are Ozempic, Wegovy, and Mounjaro?
These are brand names for the new generation of injectable GLP-1 (and GIP/GLP-1) receptor agonist medications used for type 2 diabetes and, increasingly, for medical weight management. Ozempic and Wegovy are both semaglutide (Ozempic licensed for type 2 diabetes; Wegovy for weight management). Mounjaro is tirzepatide — a dual GIP/GLP-1 agonist with even stronger weight-reduction effects in trial data. Saxenda (liraglutide) is the earlier daily injection in the same family.
These medications work by mimicking gut hormones that are naturally released after eating. They reduce appetite, slow gastric emptying, and improve glycaemic control. In trial settings, patients on semaglutide 2.4 mg weekly lose around 15% of body weight on average; tirzepatide trial data shows higher figures.
Who can use these medications in Ireland?
Per HPRA licensing and NICE clinical guidance, eligibility differs by indication:
- Type 2 diabetes: indicated when blood glucose is inadequately controlled on other medications, especially if cardiovascular disease is present
- Weight management (Wegovy, Mounjaro): typically adults with BMI ≥30, or BMI ≥27 with weight-related comorbidities (type 2 diabetes, hypertension, dyslipidaemia, obstructive sleep apnoea)
Use is not appropriate for: pregnancy or breastfeeding, personal or family history of medullary thyroid carcinoma or MEN-2, history of pancreatitis, severe gastrointestinal disease, or under-18s outside specific licensed indications. An Online Doctor will assess your suitability against current Irish clinical guidance.
Common side effects and how they are managed
The most frequent side effects are gastrointestinal: nausea, vomiting, constipation, and diarrhoea. These usually settle over the first few weeks and can be reduced by dose escalation, taking the dose with adequate hydration, and adjusting eating patterns. Other reported effects include fatigue, headache, and injection-site reactions.
Rare but important risks per HPRA-published Summary of Product Characteristics: pancreatitis (severe persistent abdominal pain warrants stopping and same-day medical review), gallbladder disease, and theoretical risk of medullary thyroid carcinoma (the basis of the MEN-2 contraindication). Regular review is essential during treatment.
Supply and cost in Ireland
Supply has been intermittent across Europe since 2022 due to global demand exceeding production capacity. The HPRA has published periodic statements on availability. Local pharmacy supply can fluctuate. Cost varies by product — Wegovy and Mounjaro are self-pay in most cases (typically several hundred euro per month at higher doses); Ozempic prescribed for type 2 diabetes is reimbursable under the GMS and DPS schemes within the indication.
How the Online Doctor pathway works
An Online Doctor consultation for GLP-1 weight-management therapy includes: structured medical history (weight history, comorbidities, contraindications), BMI calculation, weight-related cardiovascular and metabolic risk review, discussion of expected outcomes and side effects, baseline blood-test recommendation where indicated, prescription where clinically appropriate, and a structured follow-up schedule. We follow current NICE and Irish clinical-practice guidance. Book a consultation here.
Lifestyle context — the medication is part of the picture
GLP-1 medications are most effective combined with sustainable lifestyle changes. Without them, weight tends to return after stopping. Effective combinations typically include: a structured nutrition plan emphasising protein and fibre, resistance and aerobic exercise, sleep hygiene, and behavioural support. The Diabetes Ireland and HSE resources provide evidence-based programmes.
